This safety-log record applies to constructors, employers, supervisors, and competent workers responsible for Elevating work platform operations. The log is intended to prove that the relevant hazard, equipment, or site condition was checked before exposure continued.

Step-by-Step Filing Instructions

  • Identify the Controlled Work: Record the site, date, crew, equipment, area, and trade activity.
  • Check the Hazard Controls: Confirm guards, access, PPE, training, permits, isolation, or inspection evidence required for the task.
  • Sign the Entry: The maintenance and inspection record tag must include the date of the last maintenance and inspection, the signature and name of the person who performed it, and confirmation that maintenance followed the manufacturer's recommendations.
  • Store the Record: The owner must keep a permanent record of all inspections, tests, repairs, modifications, and maintenance performed on the elevating work platform under O. Reg. 213/91, s. 145(2)-(3).
